Feedback Form - OSM Township

This form is intended to be used for feedback, suggestions, compliments and/or complaints about Township programs, services, facilities or staff members.

The Township of Otonabee-South Monaghan is committed to continuous improvement and open communication with the public.
All complaints are dealt with in a respectful, transparent manner, and as quickly as possible.
It is best to address and resolve a complaint, or share a compliment at the time they occur.

All public comments are encouraged to be submitted to the Township in writing. By completing this form, all required information will be collected and utlized as required.

The Township of Otonabee-South Monaghan requests that individuals include their contact information when submitting compliments, complaints, feedback and suggestions to the Township. This will allow staff to follow up with the individual, if necessary.

All written submissions will be dealt with in a confidential manner in accordance with the Municipal Freedom of Information and Protection of Privacy Act.

Notice of Collection

The personal information collected on this form is collected under the authority of the Municipal Act, and will be used to investigate and/or follow up, as required.

Please note that this form is not intended to be used for By-law Enforcement Complaints.
